Biological age vs. chronological age:
what's the difference?
Your chronological age is fixed — it's simply how long you've been alive, and it only moves in one direction. Your biological age is a different measurement entirely: it reflects how well your body is actually functioning, and it can run faster or slower than the calendar suggests.
What actually determines biological age
Biological age is shaped by a mix of genetics, lifestyle, and environment — sleep quality, nutrition, physical activity, stress levels, and exposure to things like UV radiation or chronic inflammation all leave measurable marks on how your body is aging at a cellular and functional level.
The important distinction: while you can't change your chronological age, biological age is — at least in part — modifiable. That's the entire premise longevity medicine is built on. Two people born in the same year, with the same number of candles on their cake, can be aging at meaningfully different rates.
Biological age isn't a fixed trait you're stuck with — it's closer to a dashboard reading than a birth record. The habits and interventions that make up longevity medicine exist because that reading can genuinely be moved.
How biological age is estimated
There isn't one single test that captures biological age — it's built from multiple types of markers working together:
- →Blood-based markers. Inflammatory markers, metabolic indicators, lipid profiles, and other bloodwork that reflect how your internal systems are functioning under the surface.
- →Functional markers. Measurements like grip strength, walking speed, and VO2 max — increasingly recognised as some of the strongest real-world predictors of healthy aging, precisely because they measure what your body can actually do, not just what a single blood draw shows.
- →Body composition. Muscle mass, visceral fat, and bone density all shift with biological aging in ways that don't always track neatly with chronological age.
Why this matters more than the number on your ID
Two people of the same chronological age can have meaningfully different disease risk, life expectancy, and day-to-day quality of life — driven by how their bodies have actually aged, not by how many years have passed since they were born.
The practical value here is simple: chronological age gives you nothing to act on. Biological age gives you something to actually work toward — a number that responds to the choices you make, tracked over time rather than guessed at once.
